CRESTVIEW — A Crestview woman was charged with a DUI and three counts of child neglect following a welfare check Monday.

The Okaloosa County Sheriff's Office arrested 32-year-old Analise Guillen, according to a press release from the Sheriff's Office.

Guillen's ex-husband called authorities saying he was concerned, the release said. He had just arrived at the Destin-Fort Walton Beach Airport on Monday afternoon to deliver their child to her, but said her speech sounded slurred on the phone.

Deputies made contact with Guillen in the airport parking lot, where she was in the driver’s seat of her vehicle, disoriented and confused.

Guillen admitted to taking prescription medicines earlier in the day. She was arrested for DUI following a field sobriety test.

There were three small children in the van, leading to the three counts of child neglect.

A breath sample put Guillen’s blood alcohol level at .378, more than four times the legal limit. She was taken to Fort Walton Beach Medical Center until she could be medically cleared to transfer to the Okaloosa County Jail.
